Range-of-motion recording sheet

Graph clinician-entered joint angles with method, side, position, device and angle conventions kept separate.

How to use this tool

Use this range-of-motion tracker to understand how already measured joint-angle observations can be graphed within a consistent context.

  • Enter the joint, side, movement, measurement method and conditions for each fictional observation.
  • Record the measured signed angle in degrees and retain the date and series label.
  • Choose a compatible series and review the graph and source rows before exporting.

Fictional example and how to read the result

A fictional series may contain signed values of -5 and 0 degrees. Preserve the measurement convention; do not replace signed angles with absolute values.

Do not attempt a new examination from this page. The tracker supplies no normal ranges, diagnosis or treatment recommendation.

Retain the measurement conditions

Enter an angle obtained by a qualified clinician, including the declared zero/sign convention. Active, passive and active-assisted observations, opposite sides and different positions remain separate series.

The report shows a signed difference (last minus first) and its magnitude in degrees for each compatible group. It provides no normal range, meaningful-change threshold, camera measurement or exercise recommendation.

  • Use degrees throughout; convert other units outside this sheet before entering them.
  • Version the series label when measurement conditions change.
  • Negative entered angles retain their sign. No percentage change is calculated from an angle.
  • Observations are equally spaced in the graph, ordered by date; the table preserves every entered condition.

What to enter in each field

  • Chart series label (optional): Optional: enter an existing series label exactly to select its first compatible context for the graph. Leave blank to use the first context. All other contexts remain separate in the report.
  • Observation date: Use the date of this observation. Imported dates must be real YYYY-MM-DD dates, for example 2026-10-01. Dates order the graph; points remain equally spaced.
  • Series label: Label repeated comparable measurements anonymously. Movement, side, method, position, device, observer and angle convention all define a separate series.
  • Joint and movement: Name the exact joint and movement measured, such as elbow flexion. Different movements are not combined.
  • Side: Choose left, right, bilateral or not applicable according to the already obtained measurement. Different sides remain separate.
  • Measurement method: Choose the method used for the measurement. Active and passive readings are different contexts and are not pooled.
  • Measurement position: Describe the actual measurement position so a later reading uses comparable conditions.
  • Measurement device: Record the measurement device and relevant setup, such as a goniometer. The tool does not measure angles from a picture.
  • Observer condition: Use an anonymous observer condition, such as same assessor. Changed observer conditions are kept separate.
  • Angle / zero convention: State the declared zero and sign convention, including what a negative value means. Keep that convention unchanged for comparisons.
  • Unit: Angles are entered in degrees. No conversion, normal-range comparison or treatment interpretation is supplied.
  • Recorded angle: Enter an already measured signed angle in degrees from -360 to 360. Keep negative values when required by your convention; 0 is a measured angle, not missing.
