Fictional example and how to read the result
Scores from two different editions or measurement contexts may be incomparable even when their numbers look similar. Record the exact version.
The directory does not reproduce assessment items, provide norms or license an instrument. A score log is not an assessment or a clinical interpretation.
Visit the publisher before using an instrument
Directory links identify official resources and permission routes checked on 9 October 2026. Access conditions and versions can change; confirm them with the publisher for your intended use. Resources are not interchangeable and this directory does not select an instrument for a person.
The optional sheet logs a score already obtained under the relevant instrument’s rules. It does not administer questionnaire items, compute a standardized score, interpret a cutoff or establish meaningful change. Different forms, units, respondents, languages and conditions stay separate.
Record the exact measure name, form, version and score unit.
Confirm training, age/population, approved language and administration requirements.
Obtain permission for digital reproduction where required; a downloadable form is not an unrestricted licence.
Use the fictional worked example to try logging without entering client information.
What to enter in each field
Instrument to graph (optional): Optional: enter an instrument name from the sheet exactly. The graph selects its first matching form, unit, language and administration context. Leave blank to use the first compatible context.
Observation date: Use the date of this observation. Imported dates must be real YYYY-MM-DD dates, for example 2026-10-01. Dates order the graph; points remain equally spaced.
Instrument / entered measure: Enter the name of a measure already administered elsewhere under its publisher’s requirements. The sheet does not provide assessment questions.
Exact form and version: Record the exact instrument, form and version used. Different versions remain separate even when instrument names match.
Score unit: Enter the score unit, such as raw points or an already-obtained T-score. The tool does not convert or calculate standardized scores.
Administration / respondent: State administration and respondent type anonymously, such as self-report or clinician interview. Changed respondent conditions are separate.
Language: Record the approved language used for the administered measure. Verify translation and permission requirements with the publisher.
Comparable conditions: State other comparable conditions for interpreting the series. A changed context creates a separate graph/report group.
Already-obtained score: Enter an already-obtained score using its stated unit. Signed and decimal values are supported; 0 means a recorded score, not missing. No cutoff or meaningful-change judgement is provided.
