How to use this tool

Use this home practice planner to organize an educational example of an existing professional-agreed plan and its recorded opportunities.

  • Enter the agreed activity, planned opportunities and relevant review period.
  • Record completed practice and contextual notes separately from what was planned.
  • Review the planned, completed and reviewed entries separately and export the original worksheet.

Fictional example and how to read the result

If 4 entries were reviewed and 3 were marked Completed, the completion proportion is 75%. Entries still marked Planned are excluded; a missing review does not establish a missed opportunity.

The worksheet does not prescribe exercises, dosage or therapy. Consult the qualified professional before using any practice information.

Plan with the person, then record what happened

The activity instructions, repetitions and duration come from the user’s agreed clinical plan. This tool organizes them; it does not recommend an exercise, dose or progression.

Completion is self-report and does not establish correct performance. Planned entries are excluded from the completed-of-reviewed denominator. Attempted, changed and skipped entries remain distinct and their reasons are retained.

  • Include opportunities for choice, changing the activity and declining.
  • Leave an optional dose field blank when it is not part of the plan.
  • Use Changed when the activity or dose changes and record what changed.
  • Print the planned sheet before practice and bring the review pack to an agreed review.

What to enter in each field

  • Plan label: Name the agreed plan anonymously, such as reading practice v1. Instructions must come from an appropriate qualified clinician, not from this tool.
  • Review date (optional): Optional: choose the agreed review date; imported dates use YYYY-MM-DD. Leave blank if no date has been agreed.
  • Planned date: Enter the planned activity date; imported dates use YYYY-MM-DD. Each row is one planned practice entry.
  • Activity: Name the activity already agreed with the person and clinician. This planner does not recommend exercises or progression.
  • Entered instructions: Enter instructions from the agreed plan, including supports and when to stop or seek advice as specified by the clinician. Do not use fictional instructions as personal treatment advice.
  • Planned opportunities (optional): Optional: enter the whole number of planned opportunities from the agreed plan. Leave blank if unspecified; 0 means explicitly planned zero.
  • Entered repetitions (optional): Optional: enter clinician-agreed repetitions as a whole number. Blank means unspecified; the tool never selects a dose.
  • Entered duration minutes (optional): Optional: enter agreed duration in minutes, including decimals. Leave blank when unspecified; this is not an exercise-duration recommendation.
  • Review status: Planned means not yet reviewed and is excluded from completion percentages. Use Attempted, Completed, Changed or Skipped after review; explain a Changed entry.
  • Reported opportunities attempted (optional): Optional: enter the whole number of opportunities reportedly attempted after choosing a review status. Leave blank if unknown; 0 means a reviewed report of no attempts.
  • Change / reason / experience: Optional unless Changed: explain the change, reason, choice or experience anonymously. Self-reported completion does not establish correct performance.
