Use this goal measurement plan worksheet to practice defining an observable event and a consistent collection rule before making a data sheet.
Name a fictional meaningful activity and describe its setting and observable definition.
Specify the measurement method, observation window, baseline and collection rule in the settings and rows.
Check that another observer could apply the same rule, then export the plan and sheet.
Fictional example and how to read the result
An example definition could specify a chosen communication action during a 10-minute activity. The action and observation window must both be stated.
The plan records your definition and assumptions. A qualified professional and the person or family must decide whether any real goal is appropriate.
Define the plan before recording
This original builder assembles your wording and checks missing recording details. It cannot judge whether a goal, criterion or intervention is clinically appropriate.
The IRIS goal reference concerns educational IEPs. Use its observable condition/action/criterion/timeframe concepts thoughtfully in your own discipline; this tool does not reproduce an IRIS worksheet.
Trials and interval results use summed numerator รท summed eligible denominator. Count and duration results remain in the entered unit. Interval percentage describes sampled intervals, not percentage of time.
Agree the meaningful activity and support rules with the person.
Define one observation window and make examples/exclusions explicit.
Create a new definition version when scoring rules change; do not pool incompatible definitions.
Review the draft wording and every criterion before printing the plan.
What to enter in each field
Meaningful activity: Name an activity that matters to the person, such as preparing an art bag. This gives the recording plan its purpose.
Observable action: Write one action an observer can see or hear, for example placing an agreed material in a bag. Avoid inferred feelings or vague improvement.
Observation context: State where and when observation happens and which materials are present. Keep these conditions consistent across observations.
Allowed supports: List permitted pictures, equipment or prompts. Specify which responses count as independent and record added support separately.
Baseline description: Describe the starting observations with their dates or windows and denominator. This is entered background, not an automatically assigned baseline.
Recording method: Trials use a success count and eligible opportunities; count uses event totals; duration uses measured time; interval uses sampled interval counts. Changing this setting does not convert old entries.
Recording unit: Name exactly what is counted or timed, such as independent material placements or minutes. Keep the same unit for every row.
Denominator or window rule: For trials/intervals, define an eligible opportunity and any exclusions. For counts/duration, define the observation window so totals are comparable.
Clinician-entered criterion: Enter a criterion agreed with a qualified clinician. The tool repeats your wording and does not recommend a target or declare mastery.
Start and stop rules: Describe the event that starts observation and the event or time limit that ends it. For example start when materials are available.
Examples and exclusions: Give examples that count and do not count. Distinguish assistance, unavailable opportunities and an interrupted observation.
Observation schedule: Enter the planned frequency and context, such as two observations at the same table this week. This does not schedule appointments.
Review date: Choose the planned review date; imported dates use YYYY-MM-DD. This date is printed in the plan and is not an automatic reminder.
Definition version: Use a label such as v1. When the action, support or scoring rule changes, create a new version and keep incompatible observations separate.
Editable goal statement: Optional: enter your own complete goal wording. Leave blank to assemble a draft from the other settings; review the draft before use.
Observation ID: Use an anonymous observation label such as A or session 1. Enter one row per observation window.
Recorded numerator / value: For trials/intervals enter the observed numerator; for count/duration enter the measured total in the chosen unit. Enter 0 only for an observed zero; leave numeric fields blank when not observed.
Eligible trials / observed intervals: For trials or interval recording, enter a positive whole total of eligible trials or observed intervals, including unsuccessful ones. The numerator cannot exceed it; zero is invalid. Leave blank for count/duration.
Observation status: Observed means data were collected under the rule. Not-observed means the observation did not happen or was unavailable; leave its numeric fields blank.
Context / support note: Optional: note the actual setting or added support without names or clinical note text. A changed definition or support rule may require a separate sheet.
