Use this goal attainment worksheet to practice writing clearly distinguishable individual levels on one declared dimension.
State the fictional goal, dimension and review period.
Write all attainment levels using the same dimension and add supporting observations.
Check that levels do not overlap, then export the worksheet for discussion.
Fictional example and how to read the result
If the dimension is the number of independently recorded steps, keep every level about that count. Do not change later levels into duration or satisfaction.
The original worksheet describes entered levels. It is not a standardized assessment and does not produce a standardized T-score.
An original numeric-range worksheet, with explicit conventions
Goal attainment methods vary. This original worksheet uses five ordinal levels (-2, -1, 0, +1, +2), with 0 entered as expected attainment and baseline declared at -2 or -1. These are authored by the clinician with the person.
Use one consistent numeric change dimension and keep context, supports and measurement method constant. A bound is included at the lower edge and excluded at the upper edge, so adjacent ranges meet without overlaps or gaps.
The outer ranges extend without a numeric bound. The tool checks numeric coverage and direction, but cannot verify whether prose is mutually exclusive, levels are clinically realistic or the goal matters to the person.
Select the attained level after reviewing evidence. A measured review value can be checked against the authored range, but range membership alone is not clinical approval. No T-score, percentile, pooled score or clinical-improvement percentage is calculated.
Write original descriptions for all five levels before reviewing the outcome.
Keep the same dimension in every level; do not change distance and assistance together.
Leave the outside lower/upper bounds blank and make each interior boundary meet exactly.
Record the baseline convention, expected level 0, timeframe and review evidence.
If the goal changes, preserve this worksheet and start a separately labelled goal definition.
What to enter in each field
Personally meaningful goal: Name the personally meaningful goal agreed with the person and qualified clinician. The tool organizes an authored scale; it does not select a treatment goal.
Single numeric change dimension: Choose one numeric dimension, such as minutes of the same activity. Do not vary both assistance and duration across the scale.
Measurement unit: Enter the same unit for the baseline, review value and every range, such as minutes or metres. No unit conversion occurs.
Constant conditions and supports across all levels: State the activity, setting, supports and measurement method that stay constant across all five levels.
Agreed review timeframe: Enter the agreed review date or period, such as review in two weeks. It is printed context, not an automatic reminder.
Direction of better attainment: Choose whether higher or lower numbers represent better attainment under the authored goal. This controls the expected ordering of ranges, not a clinical judgement.
Declared baseline convention: Declare whether the measured starting value belongs at level -2 or -1 under your chosen convention. This is not a standardized score.
Measured baseline value: Enter the measured starting value in the declared unit. It must fall within the range for the declared baseline level; 0 is a measured zero, not missing.
Measured review value (optional): Optional: enter a measured review value in the same unit. Leave blank when unavailable; the value checks range membership and does not automatically approve attainment.
Clinician-selected attained level: Choose not-reviewed until a qualified clinician has reviewed evidence. Otherwise select the authored -2 to +2 level; an entered review value must agree with its numeric range.
Review evidence / interpretation: Explain the anonymous review evidence and interpretation when selecting an attained level. Do not paste identifiable clinical notes.
Ordinal level: Create exactly one row for each ordinal level -2, -1, 0, +1 and +2. Level 0 is clinician-authored expected attainment, not an automatically chosen target.
Included lower bound: Enter the included lower edge in the declared unit; a value equal to it belongs to this range. Leave blank only at the lowest numeric outside edge for unbounded below.
Excluded upper bound: Enter the excluded upper edge; a value equal to it belongs to the next numeric range. Interior boundaries must meet exactly. Leave blank only at the highest numeric outside edge.
Original attainment description: Write an original description of this numeric band under the same conditions. The tool checks numeric gaps/overlaps, not whether the wording or target is clinically appropriate.
