How to use this tool
Use this documentation review tracker to organize a fictional review checklist by item version and review cycle.
Define each review item, version and cycle before adding results.
Record Present, Absent, Not applicable or Not reviewed for each item and record pair. An omitted pair remains missing.
Review the denominator and exclusions before downloading the report.
Fictional example and how to read the result
If an item does not apply, use Not applicable. If nobody reviewed it, use Not reviewed. A missing item–record pair and an Absent result are different.
The tracker records your checklist decisions. It does not certify clinical quality, legal compliance or completeness of a real client record.
Review a sample with your own checklist
The fictional starter checklist is original and editable. Adapt it to your professional requirements with appropriate professional review; it does not reproduce a regulator’s checklist or certify compliance.
Review actual notes in your practice-approved system. This public page accepts anonymous numbers, item results and operational improvement actions, not note uploads.
Record each item once per record and cycle. Include not reviewed when the item has not been checked.
Presence denominator includes present and absent only; N/A, not reviewed and missing entries remain visible. Coverage uses each entered record × all known items for its checklist version.
Change the checklist version when item definitions change. Compare cycles only with the same declared version and item set.
Print the item-level gaps and agreed actions for the team discussion.
What to enter in each field
Review cycle: Name the anonymous review batch or period, such as cycle A. Enter all intended record-item results for that cycle to establish coverage.
Checklist version: Use a label such as original v1. Change the version when checklist wording or item definitions change; incompatible versions are not compared.
Anonymous reviewed-record number: Use an anonymous number such as 1 for the reviewed record. Keep the actual clinical document in your approved system; do not upload note text.
Original checklist item: Enter one original checklist item consistently, such as author/date identified. Repeat each known item for every record; an omitted pair remains missing.
Result: Present/absent describe a reviewed applicable item. Not applicable and not reviewed remain distinct. Blank or unentered is missing; presence percentage uses only present + absent.
Improvement action (no clinical note text): Optional: enter an operational improvement action without clinical note text, such as reviewing the local recording template. Results do not certify compliance.
