How to use this tool

Use this activity participation diary to organize educational examples of meaningful activities, recorded time, supports and access barriers.

  • Enter the activity, date, start time and recorded minutes for each fictional record.
  • Describe participation, supports and barriers without identifying a person.
  • Review recorded time and overlapping entries, then download the diary.

Fictional example and how to read the result

A record from 09:00 to 09:20 contains 20 minutes. If another entry overlaps, inspect the overlap before treating totals as separate elapsed time.

Recorded minutes and notes describe this sheet. They are not a standardized participation score or a measure of a person’s value or independence.

Describe the day in your own categories

Choose activities that matter to the person and record their setting, supports and access barriers. Experience notes are optional and do not become a clinical rating.

Time summaries add the entered durations. Unrecorded time is unknown. Overlapping entries are flagged and still retained, so totals must not be read as unique elapsed time when activities overlap.

  • Use a 24-hour start time, with duration in minutes.
  • Include rest or chosen breaks when relevant to your review.
  • Decide whether simultaneous activities deserve separate entries; inspect overlap notices.
  • This original diary is not a standardized occupational assessment.

What to enter in each field

  • Observation date: Use the actual activity date; imported dates use YYYY-MM-DD. One row represents one recorded activity episode.
  • Start time (HH:mm): Enter its start in 24-hour HH:mm format, such as 09:30. Duration must finish by midnight; split an overnight activity into dated rows.
  • Meaningful activity: Name the meaningful activity, such as making breakfast. Avoid client names or personal narrative identifiers.
  • Your activity category: Choose your own consistent category label, such as daily routines or leisure. The chart adds entered minutes by category.
  • Recorded minutes: Enter recorded activity minutes, including decimals. Zero is an actual recorded zero, not missing. Overlapping activities are flagged and totals may double count elapsed time.
  • Setting: Describe where the activity occurred, such as home or a library, without identifiable addresses.
  • Available supports: Optional: describe equipment, environmental changes or assistance that enabled participation.
  • Access barriers: Optional: describe access barriers or interruptions. A blank means no barrier note was entered; it does not prove no barriers existed.
  • Experience note (optional): Optional: record a brief anonymous experience or choice. The diary does not calculate a clinical score.
